Mumbai Doctors Preserve Hope for Teenage Cancer Survivor’s Fatherhood

In a remarkable medical intervention, a 19-year-old battling testicular cancer faced a dual battle: survival and the preservation of his future fatherhood prospects. On July 25, 2026, a dedicated team of 10 specialists at the Sir H N Reliance Foundation Hospital in Mumbai undertook a critical surgery to remove his only remaining testis, conducting an in-depth search for viable sperm cells within the nearly dead organ.

Despite the surgery being life-saving, it posed a significant threat to the teenager’s chance of becoming a biological father. The likelihood of finding any sperm in the excised testis was minimal, yet the team’s perseverance led to an unexpected success: they discovered 23 viable sperm, which were subsequently frozen for future fertility options. What started as a fight for life culminated in hope for future generations.

Background of the Teen’s Medical Journey

The teenager’s battle with health issues commenced well before his cancer diagnosis. He was born with cryptorchidism, a condition where both testes fail to descend into the scrotum. This condition can adversely affect fertility and is associated with heightened cancer risk. At the age of one, he underwent an orchiopexy procedure to reposition the testes, although irreversible damage to one testis necessitated removal. The second testis, successfully brought down, later developed significant swelling, leading to the discovery of a tumor that endangered his life.

Challenges and Innovations in Sperm Retrieval

Dr. Firuza Parikh, director of IVF at the hospital, described the situation as extremely challenging, emphasizing the team’s commitment to preserving the teenager’s fertility options. Before surgery, the teen provided two semen samples, which were analyzed using a state-of-the-art AI-assisted sperm detection system. Unfortunately, the results confirmed azoospermia—no sperm could be found in the samples.

Faced with this setback, the team decided to proceed with an innovative procedure called Onco-TESE, a specialized fertility preservation method used for cancer patients. They opted for a rare variant known as “ex-vivo Onco-TESE,” enabling sperm retrieval from the surgically removed testis outside the body in a controlled environment.

Successful Retrieval and Future Prospects

Under the guidance of surgical oncologists Dr. Raman Deshpande and Dr. Soumil Vyas, the cancerous testis was removed, initiating a time-sensitive search for viable sperm. Utilizing ultrasound and MRI scans, the team dissected small tissue fragments surrounding the tumor, navigating towards untouched regions that might still hold viable sperm-producing cells.

Embryologists, led by Dr. Varsha Kuril, meticulously examined the tissues for signs of surviving sperm, culminating in a reinforced sense of hope as 23 sperm cells were identified and cryopreserved. Dr. Parikh noted that while 23 may seem like a modest number compared to typical sperm counts, each cell represents hope, an opportunity that cancer nearly squandered. The team plans to thaw the sperm when the teenager chooses to start a family in the future.

A Milestone for Medical Innovation

Dr. Tarang Gianchandani, the hospital’s CEO, hailed the procedure as a significant achievement, marking the first instance where testicular tissue was retrieved from a heavily affected testis. The success was attributed to the collaborative efforts of the medical team, paving the way for new possibilities in fertility preservation for young cancer patients.

This case highlights the evolving landscape of cancer treatment and fertility preservation, underscoring the importance of multidisciplinary teams in addressing complex medical challenges.
